We Are Reimagining Community

The We Are One Housing Resource and Services Referral Hub is a community-centered referral pathway designed to help adults aged 60 and older, as well as other individuals experiencing housing insecurity, connect with organizations that can provide appropriate resources and services.
This Hub serves as a bridge between the church, community members, and established service organizations. Church leaders, trained volunteers and some community organizations who engage with seniors are not expected to determine eligibility, provide case management, or solve housing problems themselves. Instead, they identify individuals who may need assistance, obtain permission to make a referral, and provide basic contact information and a brief description of the housing-related need in the referral form.
Depending on the person's needs and eligibility, referrals to community service organizations may include:

Housing stabilization and housing navigation

Eviction prevention and tenant-support resources

Rental or utility assistance

Affordable and senior housing resources

Home accessibility or safety assistance

Transportation and food-support programs

Caregiver and in-home services

Health and social-service navigation

Legal and advocacy resources

Community-based programs that support aging in place
The Hub is grounded in the belief that individuals experiencing housing insecurity should be treated with dignity, compassion, patience, and respect. Particular attention is given to the experiences of older adults who may encounter ageism, technology barriers, social isolation, financial hardship, or difficulty navigating complex health and social-service systems.
The Hub allows individuals to reimagine what community looks like as it reflects the Ubuntu principle, “I am because we are,” emphasizing shared responsibility, human dignity, interconnectedness, and the importance of community in supporting older adults and others facing housing instability.
The Hub is intended for non-emergency housing, social-service, and community-resource referrals.

The Hub does not provide emergency medical care, medical advice, clinical assessment, diagnosis, treatment, medication management, or ongoing medical case management.
Individuals experiencing a medical emergency, immediate threat to their health or safety, or other life-threatening emergency should call 911 or report to the nearest hospital Emergency Department (ER). Emergency situations should not be submitted through the Hub in place of seeking immediate emergency assistance.
