​The "We Are One"
The Senior Housing Resource and Services Referral Hub serves as a pathway that empowers churches and other organizations to become a bridge for older adults facing housing insecurity.

These individuals must be:

• Age 60 or above​

• Stuggling to pay for rent (housing insecurity) 

• Currently enrolled in a Medi-Cal managed health plan

​The Hub allows individuals to reimagine what community looks like as it reflects the Ubuntu principle, “I am because we are,” emphasizing shared responsibility, human dignity, interconnectedness, and the importance of community in supporting older adults and others facing housing instability.

The Hub is intended for non-emergency housing, social supportive services, and community-resource referrals.

The Hub does not provide emergency medical care, medical advice, clinical assessment, diagnosis, treatment, medication management, or ongoing medical case management.

Individuals experiencing a medical emergency, immediate threat to their health or safety, or other life-threatening emergency should call 911 or report to the nearest hospital Emergency Department (ER). Emergency situations should not be submitted through the Hub in place of seeking immediate emergency assistance.​
